  1. Gather all your ingredients: Make sure you have everything ready.
  2. Set up your slow cooker on a flat surface near an outlet.
  3. Combine the ingredients in the slow cooker: heavy cream, sweetened condensed milk, dark chocolate chips, bittersweet chocolate chips, milk, and vanilla extract.
  4. Stir the mixture well to ensure all the chocolate chips are mixed with the liquids.
  5. Turn on your slow cooker and set it to the high setting.
  6. Allow the hot chocolate to cook for 40 minutes to 1 hour, stirring regularly.
  7. Check to ensure the chocolate is fully melted and combined with the milk.
